Privacy of Participant Records Policy


The Foundation for Medical Education and Research will not collect any personal information except when the information is specifically and knowingly provided by the participants. Examples of personal information include name, postal and e-mail addresses, phone, and fax numbers. Such information is stored and retained in the Foundation for Medical Education and Research database for the purpose of providing participant records and is not available to the general public.

Participant attendance records are maintained within the Foundation for Medical Education and Research database for the purpose of generating attendance certificates.

Participants requesting additional certificates and/or records by phone must supply identifying personal information. Faxed requests must include the signature of the person requesting the service.

Released information includes the title, date of attendance or completion and number of credits attained at the related activity.
